Brunkow, Fred Ramsdell (\u00c9tats-Unis) et Shimon Sakaguchi (Japon), pour leurs travaux pionniers sur ce qu\u2019on appelle la tol\u00e9rance immunitaire p\u00e9riph\u00e9rique, c\u2019est-\u00e0-dire les m\u00e9canismes par lesquels le syst\u00e8me immunitaire s\u2019emp\u00eache d\u2019attaquer les propres cellules du corps.<\/p>\n<p>&nbsp;<\/p>\n<p>Les d\u00e9couvertes majeures<\/p>\n<p>&nbsp;<\/p>\n<p>Shimon Sakaguchi : L\u2019identification des cellules T r\u00e9gulatrices (appel\u00e9es Tregs), un sous-ensemble de lymphocytes T, qui jouent un r\u00f4le crucial dans la suppression des r\u00e9actions immunitaires potentiellement nuisibles envers le \u00ab soi \u00bb. Cette d\u00e9couverte remet en question l\u2019id\u00e9e que l\u2019unique m\u00e9canisme de tol\u00e9rance immunitaire est l\u2019\u00e9limination des lymphocytes auto-r\u00e9actifs dans le thymus (tol\u00e9rance centrale). Brunkow et Fred Ramsdell Ils ont d\u00e9couvert le g\u00e8ne FOXP3 : un g\u00e8ne cl\u00e9 pour le d\u00e9veloppement et le fonctionnement des cellules T r\u00e9gulatrices. Lorsqu\u2019il est mut\u00e9, cela provoque des troubles auto-immuns s\u00e9v\u00e8res. Ces travaux ouvrent la voie \u00e0 des traitements visant \u00e0 moduler ou restaurer la fonction de ces cellules T r\u00e9gulatrices dans les maladies auto-immunes, dans la transplantation, et contr\u00f4lent aussi la tol\u00e9rance lors des th\u00e9rapies anti-cancer.<\/p>\n<p>&nbsp;<\/p>\n<p>Implications et perspectives<\/p>\n<p>&nbsp;<\/p>\n<p>Meilleure compr\u00e9hension de l\u2019auto-immunit\u00e9 : Ces recherches expliquent un m\u00e9canisme fondamental par lequel l\u2019organisme emp\u00eache le syst\u00e8me immunitaire de s\u2019attaquer lui-m\u00eame.<\/p>\n<p>&nbsp;<\/p>\n<p>Nouvelles voies th\u00e9rapeutiques : Le travail sur les cellules T r\u00e9gulatrices et FOXP3 inspire le d\u00e9veloppement de traitements pour les maladies auto-immunes, pour am\u00e9liorer la tol\u00e9rance aux greffes, et aussi dans le domaine de l\u2019immunoth\u00e9rapie contre le cancer.<\/p>\n<p>&nbsp;<\/p>\n<p>Un champ de recherche \u00e9mergent : La tol\u00e9rance immunitaire p\u00e9riph\u00e9rique devient un domaine central de la recherche en immunologie, avec des essais cliniques en cours pour exploiter ces m\u00e9canismes.<\/p>\n<p>&nbsp;<\/p>\n<p>Le Prix a \u00e9t\u00e9 d\u00e9cern\u00e9 par l\u2019Assembl\u00e9e Nobel de l\u2019Institut Karolinska, Stockholm.<\/p>\n<p>Le montant du prix est de 11 millions de couronnes su\u00e9doises.<\/p>\n<p>Il s\u2019agit pour Sakaguchi de travaux remontant aux ann\u00e9es 1990, tandis que Brunkow et Ramsdell ont publi\u00e9 des d\u00e9couvertes importantes autour de 2001 concernant FOXP3, et par la suite des liens entre ce g\u00e8ne et la fonction des Tregs.<\/p>\n<p>&nbsp;<\/p>\n<p>Les recherches sur les cellules T r\u00e9gulatrices (Tregs) et le g\u00e8ne FOXP3 ne sont pas qu\u2019une prouesse scientifique : elles transforment d\u00e9j\u00e0 la mani\u00e8re dont on envisage le traitement de nombreuses maladies.<\/p>\n<p>&nbsp;<\/p>\n<p>1. Les maladies auto-immunes<\/p>\n<p>&nbsp;<\/p>\n<p>Les Tregs jouent un r\u00f4le cl\u00e9 pour emp\u00eacher le syst\u00e8me immunitaire d\u2019attaquer les tissus du corps.<\/p>\n<p>Quand le g\u00e8ne FOXP3 est d\u00e9fectueux ou que les Tregs sont insuffisants, le syst\u00e8me immunitaire perd le contr\u00f4le.<\/p>\n<p>&nbsp;<\/p>\n<p>Exemples de maladies concern\u00e9es :<\/p>\n<p>&nbsp;<\/p>\n<p>Diab\u00e8te de type 1 : les cellules pancr\u00e9atiques productrices d\u2019insuline sont d\u00e9truites par le syst\u00e8me immunitaire.<\/p>\n<p>\u2192 Des essais cherchent \u00e0 renforcer l\u2019activit\u00e9 des Tregs pour prot\u00e9ger le pancr\u00e9as.<\/p>\n<p>&nbsp;<\/p>\n<p>Scl\u00e9rose en plaques (SEP) : le syst\u00e8me immunitaire attaque la my\u00e9line des neurones.<\/p>\n<p>\u2192 Des approches visent \u00e0 reprogrammer les Tregs pour r\u00e9duire l\u2019inflammation c\u00e9r\u00e9brale.<\/p>\n<p>&nbsp;<\/p>\n<p>Polyarthrite rhumato\u00efde et lupus \u00e9ryth\u00e9mateux diss\u00e9min\u00e9 : modulation des Tregs pour calmer l\u2019auto-inflammation.<\/p>\n<p>&nbsp;<\/p>\n<p>Essai clinique en cours (2025) :<\/p>\n<p>Des th\u00e9rapies exp\u00e9rimentales utilisent des Tregs modifi\u00e9es en laboratoire (CAR-Tregs) pour cibler sp\u00e9cifiquement les tissus inflammatoires \u2014 une approche inspir\u00e9e directement des travaux prim\u00e9s.<\/p>\n<p>&nbsp;<\/p>\n<p>2. La transplantation d\u2019organes<\/p>\n<p>&nbsp;<\/p>\n<p>L\u2019un des plus grands d\u00e9fis des greffes reste le rejet immunitaire.<\/p>\n<p>Les Tregs, capables de calmer les r\u00e9ponses immunitaires excessives, offrent une nouvelle voie pour induire la tol\u00e9rance sans recourir \u00e0 une immunosuppression lourde.<\/p>\n<p>&nbsp;<\/p>\n<p>Applications concr\u00e8tes :<\/p>\n<p>&nbsp;<\/p>\n<p>Greffes de reins et de foie : essais visant \u00e0 injecter des Tregs du patient pour pr\u00e9venir le rejet.<\/p>\n<p>&nbsp;<\/p>\n<p>Transplantations de moelle osseuse : les Tregs pourraient r\u00e9duire la maladie du greffon contre l\u2019h\u00f4te.<\/p>\n<p>&nbsp;<\/p>\n<p>Ces strat\u00e9gies permettraient de r\u00e9duire la d\u00e9pendance aux m\u00e9dicaments immunosuppresseurs, souvent toxiques \u00e0 long terme.<\/p>\n<p>&nbsp;<\/p>\n<p>3. Le cancer et l\u2019immunoth\u00e9rapie<\/p>\n<p>&nbsp;<\/p>\n<p>Dans les cancers, le r\u00f4le des Tregs est double :<\/p>\n<p>Ils peuvent prot\u00e9ger les cellules tumorales en bloquant les attaques du syst\u00e8me immunitaire.<\/p>\n<p>&nbsp;<\/p>\n<p>Les chercheurs cherchent donc \u00e0 inhiber les Tregs dans le micro-environnement tumoral, pour permettre au syst\u00e8me immunitaire de mieux d\u00e9truire les cellules canc\u00e9reuses.<\/p>\n<p>&nbsp;<\/p>\n<p>Exemple :<\/p>\n<p>Des anticorps ciblant FOXP3 ou les r\u00e9cepteurs sp\u00e9cifiques des Tregs sont en d\u00e9veloppement pour renforcer l\u2019efficacit\u00e9 des immunoth\u00e9rapies anticanc\u00e9reuses comme les inhibiteurs de points de contr\u00f4le (anti-PD-1, anti-CTLA-4).<\/p>\n<p>&nbsp;<\/p>\n<p>4. Les maladies inflammatoires chroniques<\/p>\n<p>&nbsp;<\/p>\n<p>Les Tregs ont aussi un potentiel th\u00e9rapeutique dans :<\/p>\n<p>-les maladies intestinales chroniques (maladie de Crohn, rectocolite h\u00e9morragique),<\/p>\n<p>-les maladies pulmonaires inflammatoires,<\/p>\n<p>-ou encore certaines dermatoses auto-inflammatoires.<\/p>\n<p>&nbsp;<\/p>\n<p>Des \u00e9quipes en Europe et au Japon explorent des traitements \u00e0 base de Tregs autologues (provenant du patient lui-m\u00eame), r\u00e9inject\u00e9es apr\u00e8s expansion en laboratoire.<\/p>\n<p>&nbsp;<\/p>\n<p>Une r\u00e9volution silencieuse en immunologie<\/p>\n<p>&nbsp;<\/p>\n<p>Les d\u00e9couvertes r\u00e9compens\u00e9es par le Prix Nobel de M\u00e9decine 2025 ouvrent une \u00e8re nouvelle :<\/p>\n<p>celle d\u2019une m\u00e9decine de r\u00e9gulation plut\u00f4t que de suppression ;<\/p>\n<p>celle o\u00f9 le syst\u00e8me immunitaire n\u2019est plus un ennemi \u00e0 combattre, mais un alli\u00e9 \u00e0 r\u00e9\u00e9duquer.<\/p>\n<p>&nbsp;<\/p>\n<p>Cette approche, plus fine et personnalis\u00e9e, pourrait transformer durablement la mani\u00e8re dont on traite les maladies chroniques et auto-immunes, notamment en Afrique o\u00f9 ces pathologies progressent rapidement.<\/p>\n<p>&nbsp;<\/p>\n<p>Le prix Nobel de M\u00e9decine 2025 r\u00e9compense une avanc\u00e9e fondamentale dans notre compr\u00e9hension du syst\u00e8me immunitaire : non seulement comment il lutte contre les agents pathog\u00e8nes, mais \u00e9galement comment il est r\u00e9gul\u00e9 afin de ne pas devenir destructeur.
