L\u2019Agence sanitaire mondiale de la sant\u00e9 a recommand\u00e9, lundi, l\u2019utilisation d’un antir\u00e9troviral injectable, le l\u00e9nacapavir, deux fois par an comme option suppl\u00e9mentaire de prophylaxie pr\u00e9-exposition (PrEP) pour la pr\u00e9vention du VIH.<\/p>\n

Selon l\u2019Organisation mondiale de la sant\u00e9 (OMS<\/a>), ce nouveau dispositif pourrait contribuer \u00e0 remodeler la riposte mondiale au VIH. Le l\u00e9nacapavir injectable offre une alternative tr\u00e8s efficace et \u00e0 longue dur\u00e9e d\u2019action aux pilules orales quotidiennes et aux autres options \u00e0 courte dur\u00e9e d\u2019action.<\/p>\n

Avec seulement deux doses par an, l\u2019injection repr\u00e9sente \u00ab une avanc\u00e9e d\u00e9cisive \u00bb<\/strong> dans la protection des personnes expos\u00e9es au risque d\u2019infection par le VIH, en particulier celles qui sont confront\u00e9es \u00e0 des probl\u00e8mes d\u2019observance quotidienne, de stigmatisation ou d\u2019acc\u00e8s aux soins de sant\u00e9.<\/p>\n

\u00ab Alors qu\u2019un vaccin contre le VIH reste hors de port\u00e9e, le l\u00e9nacapavir est ce qui se fait de mieux : un antir\u00e9troviral \u00e0 action prolong\u00e9e dont les essais ont montr\u00e9 qu\u2019il permettait de pr\u00e9venir la quasi-totalit\u00e9 des infections \u00e0 VIH chez les personnes \u00e0 risque \u00bb,<\/strong> a d\u00e9clar\u00e9 le Dr Tedros Adhanom Ghebreyesus, Directeur g\u00e9n\u00e9ral de l\u2019OMS.<\/p>\n

Baisse des financements<\/h2>\n

Ces lignes directrices sont publi\u00e9es \u00e0 l\u2019occasion de la 13e conf\u00e9rence de la Soci\u00e9t\u00e9 internationale du sida (IAS 2025) sur la science du VIH, qui se tient \u00e0 Kigali, au Rwanda.<\/p>\n

Elles arrivent \u00e0 un moment critique, alors que les efforts de pr\u00e9vention du VIH stagnent avec 1,3 million de nouvelles infections \u00e0 VIH en 2024 – avec un impact disproportionn\u00e9 parmi les travailleurs du sexe, les hommes ayant des rapports sexuels avec des hommes, les transgenres, les personnes qui s\u2019injectent des drogues, les personnes en prison ou les enfants.<\/p>\n

Parmi les autres options de pr\u00e9vention du VIH soutenues par l\u2019OMS figurent les comprim\u00e9s oraux quotidiens, le cabot\u00e9gravir injectable – qui est inject\u00e9 une fois tous les deux mois – et l\u2019anneau vaginal de dapivirine, qui font partie d\u2019un nombre croissant d\u2019outils visant \u00e0 mettre fin \u00e0 l\u2019\u00e9pid\u00e9mie de VIH.<\/p>\n

Dans un contexte de r\u00e9ductions massives du financement de l\u2019effort mondial de lutte contre le VIH\/sida – y compris du principal programme du gouvernement am\u00e9ricain lanc\u00e9 en 2003, le PEPFAR, ax\u00e9 sur la lutte contre la maladie en Afrique – l\u2019OMS a \u00e9galement publi\u00e9 de nouvelles orientations op\u00e9rationnelles sur la mani\u00e8re de maintenir les services prioritaires de lutte contre le VIH.<\/p>\n

Probl\u00e8me majeur de sant\u00e9 publique<\/h2>\n

\u00ab Nous disposons des outils et des connaissances n\u00e9cessaires pour mettre fin au sida… ce qu\u2019il nous faut maintenant, c\u2019est une mise en \u0153uvre audacieuse de ces recommandations, fond\u00e9e sur l\u2019\u00e9quit\u00e9 et aliment\u00e9e par les communaut\u00e9s \u00bb<\/strong>, a affirm\u00e9 la Dre Meg Doherty, Directrice du d\u00e9partement Programmes mondiaux de lutte contre le VIH \u00e0 l\u2019OMS.<\/p>\n

Le VIH reste un probl\u00e8me majeur de sant\u00e9 publique \u00e0 l\u2019\u00e9chelle mondiale.<\/p>\n

\u00c0 la fin de l\u2019ann\u00e9e 2024, l\u2019OMS estime \u00e0 40,8 millions le nombre de personnes vivant avec le VIH, dont 65 % en Afrique subsaharienne. Environ 630.000 personnes sont mortes de causes li\u00e9es au VIH dans le monde, et on estime que 1,3 million de personnes ont contract\u00e9 le VIH, dont 120.000 enfants.<\/p>\n

Plus positivement, l\u2019acc\u00e8s aux m\u00e9dicaments contre le VIH continue de s\u2019\u00e9tendre, avec 31,6 millions de personnes recevant un traitement en 2024, contre 30,3 millions un an plus t\u00f4t. En l\u2019absence de m\u00e9dicaments antir\u00e9troviraux, le virus du VIH attaque le syst\u00e8me immunitaire de l\u2019organisme, ce qui conduit \u00e0 terme \u00e0 l\u2019apparition du sida.<\/p>\n
